Emergency Department Discharge Instructions

The Emergency Department Discharge Instructions package provides discharge instructions to the patient on the most common healthcare issues seen in the emergency department, with information that is consistent with the Clinical Reference System content. 

Each discharge sheet includes a description, symptoms, causes, results of any test and what the patient should do after returning home, medication given and health maintenance / health promotion content, and information on when to call your healthcare provider. 

Healthcare professionals can customize, modify, or create new handouts.  Patient, provider, and organization names can be printed on each handout.
